Preparation items:

On the day of the appointment, there are several things that need to done to complete the check-in process: 

​

1. Insurance card(s) 

2. Photo Identification 

3. List of all current medication, including dosage and frequency 

4. Pharmacy name, address, and phone

5. Valid referral 

6. Co-payments/Co-insurance to be paid at the time of service

We kindly ask that you arrive to your appointment 15 minutes prior in order for us to properly register you in for your appointment. All necessary forms must be completed before seeing your physician, therefore it is critical to complete the forms before your appointment to save more time.
